Procedure
It is very easy to customize look & feel of your order forms by customizing the store template. Follow the below steps to customize the order form
1) Navigate to Home > System configuration > Store Configuration
2) Click on Edit against the order to edit the current order form you can also create new order form by clicking New order form button.
3) On the new page complete the order form as per your requirement. You can also provide the Header and Footer which will be displayed on order form page.
4) After providing all the details click on save and it will save the order form. Click on launch order form to verify the order form.
